1. Center your copies. All four cornerstones (black square in corners) must be fully intact. This requirement holds true for both sides of the form if you copy front and back.


2. READ THE INSTRUCTION NOTE ON PRINTING AND COPYING:

If you see a green pop-up note that a form must be LETTER size (as shown below), then it MUST BE printed and copied on LETTER size paper. No exceptions. The cornerstones should extend close to the edge of the sheet (as shown in 1. above) on all pages .


If you see a brown pop-up note that a form must be LEGAL size (as shown below), then it MUST BE printed and copied on LEGAL size paper. No exceptions. The cornerstones should extend close to the edge of the sheet (as shown in 1. above) on all pages.


3. Do not modify the forms using a copy machine (i.e., cutting and pasting from other forms, blanking out questions, or reducing the size of the form printed on the paper).
