Customer Satisfaction Forms

All forms are downloadable PDF files and do not require special paper! Have program participants complete the form – then mail the forms to Extension Education for processing and analysis. A summary of the results will be sent back to you.


INSTRUCTIONS FOR ALL FORMS

* Forms can be generated in two ways:

1. Print all copies on your printer.

2. Print one master copy – then make additional copies using your copier.

* It’s best to use white paper. Light color paper has worked in the past but is not guaranteed.

* Copies can be made front and back (one sheet) or front only (two sheets). One sheet is preferable.

* Complete and submit a cover sheet for each event.

* Please fill in any partially marked bubbles before sending in forms (see marking instructions at top of form).

IMPORTANT (Instructions for customizing your form):

If you only have Acrobat Reader: Enter items directly onto the form (within your web browser). Complete the entire form in one session; then print. Any changes you make to the form will not be saved.

If you have Acrobat Standard or Acrobat Professional: You have two options. You can follow the same procedure for Acrobat Reader or you have the ability to save the file to your own computer and modify it later; then print.

DO NOT ALTER FORMS. Other than entering items, any alterations will cause the forms to lose its scanning capabilities. When making copies, make sure the four square cornerstones on the front and back are clearly visible.
